For parents and guardians looking to enroll their children in WYN’s After School or Summer Camp programs, please complete a form below. If you know a child who might benefit from participation in WYN’s Community-Based Mentoring, School-Based Mentoring, or High School Success programs, please complete the Referral Form.

For school employees, concerned adults, and agencies (not including Juvenile Court) seeking to refer youth to WYN’s Community-Based Mentoring, School-Based Mentoring, After School, Summer Camp, or High School Success programs, please complete the online Youth Referral Form by clicking the button below.
